Standard / Custom Reports

Standard Reports

Standard reports are unchangeable and added to all databases and can be used to track client inquiries and admissions, ledger entries, pending signatures and diagnoses.

Inquiry Report - Shows the number of inquiries during a specific time by referral category, based on Contact Date. It displays Inquiry Date, Date of Admission, Date of Discharge, Referral Category, Referral Name and Tags.  This can be seen by month, quarter or year to date.

Admissions Report - Shows how many clients were admitted and where they were referred from. It is tracked by those with a Date of Admission. This can be seen by month, quarter or year to date.

Register/Ledger - Allows a user to display fields from the ledger and organize the information that they want to see based on categories and billing codes they are tracking.

Log Frequency Report - Allows a user to select a template and user to see how often a type of note is logged and what client the notes are being logged to. This report can be run by month and year.

Pending Signatures - This report is only seen by users who have the File Audit permission. It allows a user to see the notes that have not been signed and who still needs to sign the note for it to be completed. This report is developed based on any outstanding signatures. There is no date requirement. Also, the User drop down is populated only by those users that have an outstanding signature requirement.

TPL Diagnosis Report - Allows a user to see at a quick glance what diagnoses the company is giving, the percentage of clients given a specific diagnosis and the percentage resolved. This can be seen for all current and past clients and be narrowed to a specific Axis.

Custom Reports

Custom reports are reports that have been requested are available to all customers.

Admissions/ Discharges Report -Shows a month to month overview of who was admitted, who was discharged, what the average length of stay was and what the retention rate is over a particular year. This report can be run by a census year.

Client Birthdays - Shows client's birthdays. It is pulled off of those clients with a Date of Admission.

Client ID Cards - It is a report that will allow you to print off client ID cards. It is pulled off of those clients with a Date of Admission.

Client Picture Roster - This report pulls clients with a Date of Admission. It is a picture roster of all current clients.

Daily Census Report - Displays the therapist, client's age, parent/guardian, referrer, date of admission and estimated date of discharge/date of discharge. At the bottom there is therapist case load report. Students will appear in this report who are active between the date range specified. Status is determined by the student's Date of Arrival and Date of Discharge.

EC - All - This report shows all Educational Consultants that are related to a contact with a relationship of EC. This will show all clients that a particular Educational Consultant is related to and the client's contact information.

Incident Reports - Shows information from the incident reports that have been filled out in the Group Notes area. It can be run using a date range. Some companies may need to run this daily to report on what incidents happened the day before.

Log Status Report - This report is run by selecting the time frame, indicator, status and contact type. It will show the log entries that meet the criteria selected.

LOS by provider - Shows by provider the number of students, average length of stay, shortest and longest that their clients stay at a particular program.

Medications - Active - Shows Active Medications with a start date between the range specified

Medications - Refills  - Shows Active Medications with a refill date between the range specified

Medications - Expired
- Shows Active Medications with an end date between the range specified

Referral by Source - Admits -  Shows contacts that were referred by a specific referrer during a specific date range that were admitted. This is tracked by those contacts that have a related contact with the REF relationship and a Date of Admission.

Referral by Source - Inquiry
-  Shows contacts that were referred by a specific referrer during a specific date range. This report includes both admitted and inquiry contacts. This is tracked by those contacts that have a related contact with the REF relationship and Contact Date.

Referral by Source All - Admits
- Shows a list of all admitted referrals during a specific date range.

Referral by Source All - Inquiry -
Shows a list of all referrals whether admitted or inquiry during a specific date range.

Referred Out -Admits -  Shows which clients were referred out to another location or person during a specified date range. This is tracked by those clients that have a related contact with the REF-O relationship and Date of Admission.

Referred Out -Inquiries - Shows which clients were referred out to another location or person during a specified date range. This is tracked by those clients that have a related contact with the REF-O relationship and Contact Date.

Referred Out by Source - Shows clients that were referred out by a specific referrer during a specified date range. This is tracked by those clients that have a related contact with the REF-O relationship.

Referrer Stats - Shows a list of all referrers, how many referrals they have given, which have converted and what their conversion rate is. It is pulled from related contacts that have the REF relationship to a client.

Related Contacts - Shows all related contacts for clients with Date of Admission (Current) or Date of Discharge (Past). It will display their contact information such as phone, email, and address.  It can be sorted by the various relationship types.

Related Contacts by Tag - Shows all related contacts for clients with Date of Admission (Current) or Date of Discharge (Past). It will display their contact information such as phone, email, and address.  It can be sorted by the various relationship types and filtered by a Tag.

Staff Picture Roster - This is given to companies who request this report. It pulls all staff with the HR Access tag.

Top Referrers - Admit - Shows all referring professionals or companies who have given a referral, their contact information and the number of referrals they have given. This information is sorted highest to lowest for the selected date range. It pulls information based on Date of Admission for the client and the REF relationship for the related contacts.

Top Referrers - Inquiries - Shows all referring professionals or companies who have given a referral, their contact information and the number of referrals they have given. This information is sorted highest to lowest for the selected date range. It pulls information based on Date of Admission for the client and the REF relationship for the related contacts.

Shows all referring professionals or companies who have given a referral, their contact information and the number of referrals they have given.
